Admissions Application
The first step in joining the Akiva family is to complete the online admissions application. We look forward to getting to know you and your student!
Family Questionnaire
At Akiva, we believe that, in order to understand and get to know the whole child, we need input from those who know him/her best – his/her family. Please complete this form to submit our Admissions Office.
Teacher Recommendation
For Kindergarten applicants only
Teachers have special insights as to how children learn and their developmental process. Please provide your child’s teacher with this form and have him/her send it back directly to our Admissions Office.
Please have your child's teacher submit the completed form to Celia Lerch, Akiva Director of Enrollment & Outreach.
Teacher Recommendation
For 1st-6th grade applicants only
Teachers have special insights as to how children learn and their developmental process. Please provide your child’s teacher with this form and have him/her send it back directly to our Admissions Office.
Please have your child's teacher submit the completed form to Celia Lerch, Akiva Director of Enrollment & Outreach.
Release of Records
As part of our application process, we require transcripts, test scores (if applicable), evaluation results (if applicable), and other pertinent information from you child’s current and past school(s). Please complete this form for submission to your child’s current school.
Independent School Entrance Exam (ISEE)
Akiva requires ISEE (Independent School Entrance Exam) scores for our Middle School applicants. You can find information about where to take the test here.
